INCIDENT REPORTING

When an incident arises at the chapter causing harm, injury or property damage to any person, the following information must be obtained immediately. Do not withhold reporting an incident to obtain required information. Chapters/Affiliates are expected to submit this form within 24 hours of the incident. Your submission is automatically sent to the Executive Director and is only shared with individuals directly involved in the incident and our insurance provider. 

Please help us eradicate sexual assault, hazing and other dangerous/risk behavior by reporting incidents that you feel may be inappropriate. We ask that you provide your name and contact information, so we can follow up with you concerning the allegation. Staff will keep your identity confidential. Your contact information will keep people safe by allowing us to conduct a more thorough investigation.  

Alpha Delta Phi has a Medical Amnesty Policy which requires calling 911 if someone needs medical attention without fear of personal or chapter disciplinary action. Thank you for assisting Alpha Delta Phi in our efforts to eliminate hazing from our organization.

HAZING PREVENTION

The Risk Management Policy of Alpha Delta Phi states: “No chapter, affiliate, member, Associate Member or alumnus shall conduct, participate in nor condone hazing activities. Permission or approval by a person being hazed is not a defense.”

Hazing is the antithesis of everything that we believe in as an organization and Alpha Delta Phi is committed to staying true to its values. Below you can find a list of resources to help combat hazing, educate yourself and others, and report hazing when you see it taking place.

The Hotline provides an anonymous telephone line for anyone to report a suspected or recent hazing incident to one number 1-888-NOT-HAZE (1-888-668-4293) that accepts calls 24 hours a day.

HPO is a national organization dedicated to empowering people to prevent hazing, by providing education and resources, advocating on hazing prevention, and building partnerships with others.
